Septorhinoplasty

Septorhinoplasty

Septoplasty / Nasal Airway Reconstruction

Septorhinoplasty Overview

Septorhinoplasty is a functional procedure used to correct structural abnormalities of the nose that affect breathing. It may involve straightening the septum, supporting the nasal valves, or reconstructing weakened areas of the nasal framework. In some cases, additional cartilage is required to provide structural support, using either autologous costal cartilage or donor costal cartilage.

This information outlines common indications, the general surgical approach, and typical postoperative considerations.

General Indications for Septorhinoplasty

Septorhinoplasty may be considered for patients who have persistent nasal obstruction or structural issues such as:

  • Nasal obstruction: Chronic nasal congestion or difficulty breathing due to structural abnormalities such as a deviated septum, collapsed nasal valves, or nasal tip deformities.
  • Traumatic injuries: Nasal fractures or injuries resulting in nasal deformities that affect breathing or nasal function.
  • Congenital abnormalities: Birth defects or structural abnormalities present from birth that interfere with nasal breathing.
  • Revision surgery: Correction of previous rhinoplasty procedures that have resulted in functional or aesthetic complications.

In Australia, Medicare and some health funds may contribute to the cost of Septorhinoplasty if strict functional criteria are met. One such criterion is the “NOSE” score, which assesses the severity of nasal obstruction and functional impairment. The components of the “NOSE” score may include:

  • Nasal congestion or blockage
  • Nasal obstruction or trouble breathing
  • Sneezing or runny nose
  • Emotional distress caused by nasal symptoms

Patients with a sufficiently high “NOSE” score may be eligible for Medicare and health fund contributions towards the cost of functional rhinoplasty. Dr Griffin will assess your NOSE Score during your initial consultation.

Septorhinoplasty Operation

During a Septorhinoplasty, Dr Griffin will perform the following steps:

  • Anaesthesia: You will be placed under general anaesthesia.
  • Incisions: Depending on the specific issues being addressed, Dr Griffin may make incisions inside the nostrils (closed rhinoplasty) or across the columella (open rhinoplasty) to access the underlying nasal structures.
  • Correction of functional abnormalities: Using specialised instruments, Dr Griffin will reshape or reconstruct the nasal structures to improve nasal airflow and function. This may involve straightening a deviated septum, reinforcing weakened nasal valves, or augmenting the nasal dorsum or tip.
  • Graft placement: In cases where additional support or volume is needed, Dr Griffin may use autologous (from your own body) or donor costal (rib) cartilage grafts to augment or reconstruct the nasal framework.
  • Closure and recovery: After completing the necessary adjustments, Dr Griffin will carefully close the incisions, and nasal splints and a cast may be used temporarily to support the newly reshaped nose and promote healing.

Recovery After Septorhinoplasty

Recovery from Septorhinoplasty typically involves the following:

  • Nasal congestion and discomfort: You may experience nasal congestion, mild discomfort, or swelling following the procedure. Dr Griffin will provide instructions for managing these symptoms, which may include nasal saline rinses and pain medication as needed.
  • Activity restrictions: Rest is essential during the initial recovery period. Avoid strenuous activities and heavy lifting to prevent complications and promote healing.
  • Follow-up care: Dr Griffin will schedule follow-up appointments to monitor your progress and ensure proper healing. It’s important to attend these appointments and communicate any concerns or symptoms you may have.
